Repeat Medication Reviews

All patients on repeat medication and/or have long term conditions will notice that we are changing the way we review your care. We will be inviting you for a review on the month of your birth. This will be an opportunity to discuss your medication, any results from blood tests or hospital appointments and to discuss the future management of your care.
